Before your child can begin 7th grade, he/she must have documentation of having received all required school-entry immunizations, including a Tdap booster after the age of 7. 

A Tdap booster is for tetanus, diphtheria, and pertussis. 

Documentation (Certificate of Immunization) of your child receiving the Tdap booster needs to be provided before the first day of school or your child will not be able to attend per Florida Law.
 

A valid Religious Exemption from the Florida Department of Health (Form 681) is also acceptable to attend Florida public school.

Childhood vaccines, including the Tdap booster, are free at the Department of Health in Charlotte County and require an appointment:  941-624-7200
